Contributing to the dispute of Marfan syndrome in Abraham Lincoln, in 1964 a physician published his findings and observations in the Journal of the American Medical Association which diagnosed Abraham Lincoln that he had Marfan syndrome (Kugler). But despite the false accusations, Marfan syndrome does exist in this world. Anyone who inherits Marfan syndrome faces the real risks and threats which contain devastating consequences. For example, a hard blow to the upper chest can result in immediate death because the connective tissues are alarmingly weak which can lead to serious heart complications and internal bleeding (Schnitzer 198). Abnormalities in the skeleton, heart, and eyes all contribute to the characterization of Marfan syndrome (Schnitzer 198). Any affected or damaged connective tissues can create or cause internal complications, especially concerning with the heart and the aorta (â€Å"Marfan Syndrome†), which is the most serious life threatening risk that can prove to be fatal (Jay). Marfan syndrome is an inherited disorder, characterized by several malfunctions including long bone overgrowth and several abnormalities of the skeleton, heart, and eye, which is caused by a defect and negative mutations in the fibrillin gene or fibrillin-1 that can weaken and devastate the connective tissues throughout the body guaranteeing further complications. In other words, Marfan syndrome weakens the connective tissues, augmenting the risk of damage in bone overgrowth, the skeleton, heart, and eye and at the same time affecting multiple crucial systems throughout the body. Marfan syndrome is a no laughing matter, according to four significant reasons: its expanded history, problematic causes, genetic description, and current research. First, Marfan syndrome was carefully observed in the late 1800 century. In 1896, a young patient was meticulously examined by Antoine Bernard – Jean Marfan, hence the name Marfan syndrome, who observed and first described the disorder (Keane). Marfan observed that the young patient had developed long, thin digits – such as the fingers, thumbs, and toes on the hands and feet – as well as the development of unusual long limbs (Keane). Little did he know, however, that this disorder was caused by mutations in the fibrillin gene that encodes a significant component, connective tissues (â€Å"Marfan Syndrome†). But the cause was eventually discovered and identified by researchers in 1991, with the help of a generous foundation, the March of Dimes (â€Å"Birth Defects†), which is the second reason why Marfan syndrome is a serious matter. Fibrillin is a protein found in different areas of the body such as the aorta, ligaments, bones, and the lungs (â€Å"Birth Defects†). Thus, fibrillin helps regulate a transforming growth factor called Beta that plays a crucial role in tissue growth and repair (â€Å"Birth Defects†) which helps stabilizes many significant areas throughout the body. Genetic description becomes the third reason. Marfan syndrome can be inherited by anyone (â€Å"Marfan Syndrome†), making it autosomal dominant (Jay). Autosomal dominant basically means that one parent can be diagnosed with the disorder (â€Å"Marfan Syndrome†) and have a 50-50 chance of passing it on to their children (Jay). The bottom line is that Marfan syndrome is a dominant genetic trait and that everyone can inherit it under certain circumstances (â€Å"Birth Defects†). Finally, being the fourth reason, current research is still underway, despite the fact that there is no cure yet. However, beginning in the year 2007, a clinical trial began comparing the effects of two different medicines in the effort to reduce the rate of progression of aortic complications (â€Å"Birth Defects†). Unfortunately, studies show that about 25 percent of cases dealing with Marfan syndrome are sporadic, or in other words, caused by a new mutation (â€Å"Birth Defects†). Affecting as many as one in every 10,000 to 20,000 people (â€Å"Donohue†), Marfan syndrome is one of the most common genetic abnormalities. Despite the fact that a cure has not been discovered yet, medical researchers as well as doctors are striving their best to prevent or slow the symptoms of Marfan syndrome and to reduce the complications as well. Marfan syndrome comes with some unfortunate factors such as the signs and symptoms, test screening and diagnosis, and the prognosis also known as the outcome. First, signs and symptoms are crucial to identify as soon as possible. The symptoms of the disorder include, but are not limited to, flat feet, chest that sinks in or sticks out, thin and narrowed face, small lower jaw, nearsightedness, dislocation in the lenses of the eye’s, learning disability, and scoliosis or when the spine curves to one side (Board). However, identifying the symptoms of the disorder can prove difficult (Jay) since age is a crucial factor involved (â€Å"Marfan Syndrome†) and that the establishment of a specific, formal test still does not exist (Jay). In other words, doctors become aware of this disorder in people only when the noticeable signs occur caused by the disruption of the connective tissues. Also, the disorder is present at birth even though it may not be diagnosed until at a later time (â€Å"Marfan Syndrome†). But when the symptoms do occur, the hands, fingers, feet, and toes may be unusually long as well as the tallness in height with long, thin arms and legs (Schnitzer 198). Skeletal abnormalities include a few deformities of the chest and face (Schnitzer 198). Other symptoms deals with the eyes, such as the dislocation of the eye’s lenses (â€Å"What is†), and the lungs, which can increase the risk and trigger asthma, bronchitis, pneumonia, and collapsed lungs (â€Å"Marfan Syndrome†). Secondly, the diagnosis of this syndrome can be determined by some medical evaluation. Based on a clinical diagnosis, the heart, eyes, blood vessels, spine, and skeletal system are most likely to be checked out by medical professionals (â€Å"Marfan Syndrome†). When diagnosing a disorder, machines and methods are used to receive valuable information about the status of it. For example, to monitor the changes in the heart and blood vessels, chest x-rays, an electrocardiogram (ECG) and an echocardiogram – which is a type of machine that uses high frequency sound waves to produce a moving picture – all may be used in the process (â€Å"Marfan Syndrom†). Thirdly, the outcomes may seem very challenging to those who have the disorder, but there is a feasible way to live through it all. Changes in the heart and blood vessels were developing in about 90 percent for those who inherit the disorder (â€Å"Marfan Syndrome†). Because of this change, people must restrain from heavy activities and exercise as well as strenuous competitive sports which can all place high stress on the heart and aorta (â€Å"Birth Defects†). Thus, participation in less vigorous activities – such as walking, golfing, swimming, and slow-paced tennis – was consider safer to do (â€Å"Birth Defects†). But despite the helpful guidance and support of many programs, treatments, and changes, 50 is usually the average age of death for those who sadly have the disorder (Schnitzer 198). Heart complications and failure is usually a cause for this. However, there have been records and reports where patients have survived beyond the age of 50, well into their 60s (Board). Despite whether the factors are negative or not, inheriting this type of disorder requires immediate attention and possibly significant changes as well. People who inherit Marfan syndrome will most likely face devastating changes due to the many negative complications and the risks factors that follows. Concerning with the complications, there are four main areas where the disorder can create devastating results which include the heart, face, eyes, and chest. Heart malfunctions is the first one, which is the most serious not to mention the most deadly complication of all (Jay). This is because the aorta, connected with the heart, could easily rupture due to enlargement (Jay). Cardiomyopathy, which is basically the enlargement and the weakening of the heart muscle, is another problem because this could lead to heart failure (â€Å"Marfan Syndrome†). But the bottom line for the heart complications associated with the disorder is that the large blood vessels connecting to the heart may gradually weaken and expand, which can eventually rupture causing death (Schnitzer 198). Complications in the eye, such as nearsightedness and the dislocation of the lenses, can be corrected with glasses or contact lenses; however, more advanced methods may include the usage of medical lasers (â€Å"Birth Defects†). Other complications involve with the chest – in which it may protrude outward or inward – joints – where it becomes increasingly flexible – and the face – where patients may have a long, narrow face and the roof of the mouth may be higher than normal (â€Å"Marfan Syndrome†). Risk factors are another concern, closely following the complications of the disorder. For example, activities that demand such strength and vigorous energy should be avoided at all cost for those who inherit the syndrome because of the following risks, capable of damaging the aorta and eyes (Jay). When someone discovers that a family member has Marfan syndrome, it would be the best course of action to visit a genetic counselor to discuss and ascertain who else have or may have inherit the syndrome. Another example involves with pregnancy. If a woman inherits the disorder and becomes pregnant, not only is she at great risk of her aorta rapidly enlarging, causing it to burst resulting in death, but she is also at risk of passing the inherited disorder to her newborn (â€Å"Birth Defects†). Thus it is considerably wise when one or the other parent inherit the syndrome speaks to a genetic counselor and avoid pregnancy to avoid death in return. Despite the numerous complications and risk factors associated with the disorder, there are many feasible solutions and methods on better controlling the outcome of the situation and matter. Despite the many complications associated with Marfan syndrome, there are several ways to fight back and come up with positive solutions using the advantage of special treatments and knowing how to prevent it. First, knowing how to fight back the disorder can be helpful. Some will need regular, medical check-ups, while other may need surgery. Otherwise, three significant steps can be taken to either detect or monitor the disorder thus preventing further complications. One, a medical examination by a cardiologist can examine the heart and other vital organs (Jay). Two, examination of the eye to check the position of the lens on a regular basis can prove crucial (Jay). Three, the participation of genetic counseling is recommended where patients are informed about the risks of passing the disorder on to children and are given advice to deal with the disorder (Jay). Another way to treat the syndrome is using beta blockers, which are high blood pressure medications where they help reduce the strength and frequency of heartbeats (â€Å"Birth Defects†). Surgery is the last defense against the disorder for many. Surgery is only required when the aorta dilates or stretches too much to the extent where necessary repairs need to be made to avoid extensive internal damage (â€Å"Birth Defects†) which can be fatal. Secondly, prevention is harder than it looks when it comes to Marfan syndrome, since the symptoms usually begins at a very early age in either childhood or adulthood. Thus, there is really not a successful way to prevent the disorder (â€Å"Birth Defects†). However, if the diagnosis can be detected at an early age, there is the opportunity where help and treatment can increase the chances of avoiding serious complications (â€Å"Birth Defects†). Although there are several feasible solutions and treatments that can be used to the full extent in treating the disorder, the many fatal risks are still present, and alarmingly high. The one, inherited disorder where connective tissues are consequently very weak caused by the defect in a gene called fibrillin-1 which results in many internal organ complications – such as the skeleton, heart, and eyes as well as the development in the long thin limbs and digits – is Marfan syndrome which can become increasingly deadly if not given the proper treatment it deserves. People who have the disorder suffer physically, internally, and mentally. Physically, they can develop skeleton abnormalities, changing the way how they look, and develop long, thin digits making them look unusually tall and thin. Internally, people can suffer all kinds of damage in the heart, eyes, skin, bones, blood vessels, the aorta, and the lungs. The disorder mentally affects both the patient and their relations with others, forcing change in their lives. Even though a cure for the disorder has not been successfully discovered, treatments and versatile methods have been introduced throughout the years since its discovery by Marfan himself. And, researchers today are still striving hard through their own success, failures, and teachings, hopefully opening up the gates to a cure of this devastating disorder, Marfan syndrome. There are three ways in which women are discriminated against: in the workplace, television, and in politics. According to Napoleon Bonaparte â€Å"Nature intended women to be our slaves they are our property. Woman are nothing but machines for bearing children. Even though Napoleon is dead, and has been for a while now, this way of viewing women is very much alive. Why is it that woman are paid less than men for doing the same jobs? For example, a male professional basketball player gets paid way more than a woman. A 2008 study of 63 countries shows that a woman in the same job as a man with the same responsibilities is paid 10% to 15. 6% less then the man. Television plays a big part on the way that our country views woman. (Buckler,13) According to the Center for Disease Control; (CDC) there are approximately five million people in the United States that have heart failure. Approximately 550 thousand people are diagnosed with heart failure each year. African Americans have a higher death rate than Caucasian Americans; the estimated medical costs in 2006 were 29. 6 billion dollars. Heart failure has a very broad range of causes, some of the more common cardiovascular causes are: congenital heart conditions, valvular disease, coronary artery disease, myocarditis, cardiomyopathies, myocardial infarction, and hypertension. Noncardiovascular causes of heart disease include pulmonary embolism, thyrotoxicosis, and chronic obstructive pulmonary disease. (Williams,407 ) Lifestyle choice such as smoking, excessive alcohol intake and environment are contributing risk factors. (Buckler,13-15: Williams,408) The symptoms will vary depending on which side of the heart is failing, patients age, and the severity of the disease. However fatigue and weakness are the hallmark signs of heart failure. Williams,483) When pump failure occurs in a damaged left ventricle, referred to as left-sided heart failure (most commonly occurs here); signs and symptoms can include: Dyspnea on exertion, Cheyne-Stokes respirations, crackles, wheezing nocturia, restlessness, cough, pink frothy sputum, weakness, and paroxysmal nocturnal dyspnea, and cyanosis. Right ventricle failure or right-sided heart failure can either be primary failure or secondary to left-sided heart failure. Signs and symptoms can include: Peripheral edema, ascites , abdominal pain nd fullness, anorexia or sudden weight gain, tachycardia, jugular vein distention, nocturia, and nausea. (Williams,484) Controlling modifiable factors can prevent heart failure, such as smoking and diet. However these factors do not directly cause heart failure but is secondary to an underlying disease such as coronary artery disease that can lead to heart failure. In general heart failure patients have a poor prognosis unless the cause is correctable. If the heart failure is primarily due to a valvular disorder, repair or replacement could reverse the disorder. Heart transplant may be an option for some patients. Several diagnostic procedures may be done to aid in the diagnosis of heart failure. First a complete history and physical examination will be performed, followed by one or more of the following: chest x-ray to detect enlarged heart and pulmonary congestion; electrocardiogram (ECG) is used to assess hypertrophy, ischemia, and arrhythmias; echocardiograms assess blood flow and left ventricular function; cardiac catheterization may show ventricular dilation, coronary artery occlusion, and valvular disorders. Serum blood testes include: Brain natriuretic peptide (BNP) is very specific for detecting heart failure. Electrolytes, blood urea nitrogen, creatinine (to rule out kidney disease); complete blood count (CBC) is used to rule out anemia and infection; Arterial blood gasses (ABGs). (Myers, 484) These are just a few of the main diagnostic tools used to detect or eliminate heart failure and determine the cause. (Buckler,15-17) Treatment is patient specific and will focus on the underlying cause and severity of the heart failure. Pharmacologic interventions may include; ACE inhibitors to decrease cardiac hypertrophy and afterload, these are the first line of pharmacologic interventions. Diuretics are used to decrease fluid overload, beta blockers are used to improve cardiac output, and an anticoagulant may be needed. These medications along with cessation of smoking, close and accurate daily weight monitoring, low sodium diet to help eliminate fluid retention, a high potassium diet may be needed if a thiazide diuretic is prescribed, regular light activity with appropriate rest periods, within the parameters set by the physician, and oxygen therapy. Myers,487-89) Before discharge it is imperative that the patient and family understand the purpose, dosage, schedule, and route of administration of all the medications they are taking, as well as the side effects that need to be reported to their physician. Avoid any over-the-counter medications that may contain sodium, and if they are unsure of a medication, cons ult the physician or pharmacist. They need to understand the level and importance of daily activity and when to stop exercising and when to report to their physician; what procedure should be done when assessing their daily weight and the importance of notifying the physician of daily weight gain of two to three pounds of weekly gain of five pounds. They should also know the importance of the low sodium diet; or high potassium diet if their on a thiazide diuretic, and maintain their fluid restriction. (Buckler,19-20) Supportive resources should be provided to the patient and family. Any knowledge deficit in these areas can have dangerous consequences. Beers, 657-68 ) Patients will often experience shortness of breath, which will increase anxiety. They need to know that the anxiety can exacerbate the shortness of breath. Teaching the patient to sit in the Fowlers position, how to use oxygen as prescribed will help decrease the demand of oxygen. (Beers, 657) Patients should be aware of the different relaxation therapi es that they can practice to help alleviate stress and help slow their breathing; they need to know how and when to use any inhalers they may have prescribed, and the signs and symptoms of any side effects. I choose to research heart failure because of the amount of people that have the disease, its devastating effects on a person lifestyle, and the many underlying diseases that can lead to the disease. Heart failure may not be curable in most cases, however with a strict adherence to the medication, exercise, diet, fluid restriction; a patient has a chance to stop the progression of the underlying cause, therefore avoiding heart failure. Locke maintained that the state of nature was a happy and tolerant one, that the social contract preserved the preexistent natural rights of the individual to life, liberty, and property, and that the enjoyment of private rights the pursuit of happiness led, in civil society, to the common good. Lockes form of government is simple, yet confusing. Lockes government is broken down into four main areas, the State of Nature ( SN ), the State of War ( SW ), Civil Society ( CS ), and Political Society ( PS ). Locke begins by recognizing the differences between power, in general, and political power in particular. Locke believes political power to be, the power of a magistrate over a subject. (2) The subject remains under the magistrates rule by choice. This brings about the State of Nature. The SN is a state of perfect freedom, no one is controlling others and no one is being controlled, everyone is equal. Locke comes to say that the only way someone can rule over us is if we let them. By doing this we are not abandoning our SN, but remaining in it. It is ones choice to let another preside over them. Our SN is threatened though because we do not have complete control, therefore we come into the State of War. Under SW we have taken away others SN or given up our own. For us to get it back we come into Civil Society. By lending out our SN we come together to protect it. We are given back our SN after it has been restored. We are no longer threatened by someone taking it away. The problem that arises is the fact that this is not a very solid solution. This leads to the Political Society. People agree to get together and establish a PC (AKA government) The PC is responsible for protecting others. We are still in our State of Nature as we have lended it out, received it back and come to terms with others in arranging a Political Society. Locke is attempting to understand the proper relationship between a people and a government. Jeffersons ideas are very close to those of Lockes. Which proves Lockes work had an impact on him. Behind him seated at the back table is a woman in a flowered straw hat who is drinking from a glass, she looks directly out of the picture plane past the table in the foreground at the viewer. A man seated to her left is barely visible in profile and past the shoulder of the man in the cream colored jacket. The entire scene is brightly lit with ambient light from the open sides of the porch the majority of which were enters from the front left of the picture plane. The white tablecloth on the table in the foreground glows with reflected light and the glasses and bottles on the table twinkle in the light. Part IV – Examination of the Art Objects Medium/Technique/Process Used: The medium used is this painting by Renoir is oil on canvas. The work was painted en plein air (outside) utilizing the alla prima (wet on wet) technique and also the classical layering technique of oil painting employed by the old masters. In these processes the first step would require the preparation of a surface on which to apply the oil paint. A linen canvas would be stretched around a wooden frame and tacked into place. The surface of the canvas would then be protected from the acidic qualities of the oil paint by applying layers of rabbit skin glue and chalk known as a ground. Next a primer of white lead paint or glue mixed with chalk â€Å"gesso† is added to provide a base layer for the painting. A rough outline of the scene would then be sketched onto the canvas as a guide for painting. Next the creation of a palette from which to paint from would require selecting the proper pigments. (Eastlake pg.234) For the portions of this work that were painted en plein air in the alla prima technique in the Impressionist style such as the tableware in the foreground Renoir would have used portable tubes of pigmented paint. Other portions that were executed in his studio employed classical oil painting techniques of layering colors and then glazing which creates a lustrous glow and provides the three dimensional quality of modeling. This technique is evident in the modeling of the forearm and the hand holding the cigarette of the man seated at the table in the right foreground. Colors would then be applied to their proper regions according to the sketch starting with the darkest and ending with the lightest, a mosaic upon which the details and highlights could then be painted. Finally upon completion the painting would be sealed with a varnish to protect its surface. (Sanders pg.11) Part V Examination of the Art Objects Composition (its Form Organization): 1. Identify and describe a specific incidence of composition from the list found below: A. Line: Renoir uses a sharp clean line to define the arm of the man seated at the table in the right foreground. The precise outline of the arm adds to the contrast of color between the arm and the man’s white shirt, the white tablecloth, and chair adding perceived depth to the shape of the arm. In contrast Renoir’s use of line in creating the marsh grasses outside the balcony is loose and diffused, giving the grass a perceived movement and depth. B. Shape: The semi circular shape of the awnings scalloped edge is distorted into curving points giving the illusion that the awning is swaying in the breeze adding a sense of movement to the air that is picked up by the grasses beyond it. C. Space: The open space left of center, framed by the two men in white tank tops allows for an unobstructed view of the landscape beyond providing perspective and contrasts the densely populated right side of the picture adding to the intimacy of the assembled group. This open space also directs the viewer gaze to the centerpiece of the table which is the symbolic heart of this painting. D. Color: Applications red are spread about the painting accenting the amorous qualities of the subjects depicted. Most evident in the women in varying profusion and intensity, the color red highlights the garments at the cuffs and necks, in the flowers they wear on their hats, and on their lips. Although more subtle, accents of red are present on the men as blushes to their face or on their lips. The most overtly amorous male in the right to the back wears a shirt and hat both striped with red. The most overtly sexual female seated, at the table in the left foreground has red tracing her neck and an explosion of red in the flower on her hat. Finally the red striped awning with its tong like flaps covers the entire flirtatious scene. E. Texture: Renoir employs variations in color, shadowing, and white highlights to render the textural appearance of the straw hat worn by the man seated at the table in the right foreground. F. Pattern: The diffused lines of blue/green create a layered pattern in the marsh grasses outside the porch rendering the effect of overlapping leaves with combined density. G. Time and Motion: The motion in this painting is imparted by the marsh grasses and awning gently swaying in the breeze. The aspect of time slowly passing is expressed through the sailboats gliding along the river and in the leisurely poses of the subjects along the railing. 2.
